Why use this generator
Works with any Microsoft Forms link
Paste the short forms.office.com/r/… link or a full forms.office.com or forms.cloud.microsoft URL — all encode correctly.
More than plain black and white
Microsoft Forms' built-in QR code is a basic PNG. Here you can change colors, put your school or company logo in the middle, and choose a dot style.
SVG for posters and banners
Download a vector SVG that stays sharp at any print size, or a PNG for slides, handouts, and Teams posts.
Nothing leaves your browser
The link is encoded locally — we never see your form or its responses.
Tips before you print
Check who can respond
If the form is set to 'Only people in my organization', anyone scanning from outside has to sign in with a work or school account. Switch to 'Anyone can respond' for public events and customer feedback.
Test on a phone that isn't signed in
Scan the code with a personal phone before printing so you see exactly what a guest sees, including any sign-in prompt.
Editing questions doesn't break the code
The share link stays the same when you add or change questions, so a printed code keeps working. Only a brand-new form gets a new link.
Frequently asked questions
How do I create a QR code for a Microsoft Form? +
In Microsoft Forms, open your form, click Collect responses (or Share), and copy the link. Paste it into this generator's Website/Text field, adjust the colors or add a logo, then download PNG or SVG.
Doesn't Microsoft Forms already make a QR code? +
Yes — the share panel has a QR code option that downloads a plain black-and-white PNG. Use this tool when you want brand colors, a logo in the center, or an SVG file for large prints.
Do responses still go to my Microsoft account? +
Yes. The QR code only contains the link; submissions go straight to Microsoft Forms as normal, and we never see them.
Do these QR codes expire? +
No. It's a static QR code that stores the link directly, so it keeps working for as long as your form is open to responses.
Can I see how many people scanned it? +
Not with a free static code. Microsoft Forms shows how many people responded; counting scans needs a dynamic QR code on a paid plan.
Reusing the same poster for a new form each term?
A dynamic QR code lets you point the printed code at a different form whenever you like, and counts every scan — no reprinting needed.
